Job Description Summary: The practice puts patients first – using creative team-based care models, multi-disciplinary integration and a unique payment model that free the physician and care team from the constraints of traditional medicine.  The practice will provide patients with open access, multi-modal communication (email, phone, web encounters), deep education and personal health care management and coaching.  The practice is built as a community collaborative, based on best practices around the country. 

The Medical Director will provide leadership for a team of clinicians comprised of 2 physicians and 1 nurse practitioner. He or she will act as a creative partner as we continue to evolve this new practice and expand the care model within the community.  Physicians in the practice will be able to spend more time with patients, have smaller panel sizes, have more team-based support, have less administrative and billing requirements and will be leaders in the evolution of this community initiative.

Complex Care Medical Director role would include:

  • Lead Physician:  Have their own panel of patients to practice in this innovative and robust model.
  • 75% clinical and 25% administrative responsibilities.
  • Care Model Partner: Lead the refinement of the care model and set the tone for the whole team. Work with the community collaborative team to continue to build the service model and expand the Center’s services.
  • Community Leader: Be a liaison to community physicians, advocating and educating about the practice and the model.  Working with the community to improve the partnerships and clinically support the design of transitions.
  • Clinical Team Leadership:  Lead a team of clinicians comprised of 2 physicians and 1 nurse practitioner.
  • Clinical Team Training:  Train and mentor clinical team as needed.
  • Quality Leadership:  Provide quality oversight for initiatives and quality measures.
  • Communications: (in and beyond the community) Use the power of voice and knowledge to present the model for different audiences.
  • Expansion Opportunities:  Future goals include the opportunity for this model to expand.
     

Experience: Candidates will be a board certified Internal Medicine or Family Practice Physician with 5+ years of experience successfully managing complex care. “Out of the box” (“or without a box”) thinker, who has leadership experience and creative experience outside of medicine (ie:  new products, customer service or technology nimbleness) is desired. Successful candidate must have strong communication skills. Leadership experience gained in community medicine, teaching or the military would be a plus. Of particular interest would be VA Physicians and Medical Directors at other FQHCs.
