Description Job/Practice Description: Evaluation, treatment and disposition of all patients who present to the emergency department including writing short term orders on patients that are admitted. This support includes such activities as evaluation and triage, ordering lab and radiology tests, prescribing medications, emergency wound care, wound closure and suturing, suture removal, emergency airway management, hemodynamic stabilization of patients prior to transfer or admission, joint needle aspirations, epistaxis management, changing of gastrostomy and suprapubic tubes, drawing venous blood, performing arterial punctures for blood gas testing, insertion of chest tubes, Cooke catheter, nasogastric tubes, peripheral and central venous lines, aspiration of cavities-thoracentesis and paracentesis, incision and drainage of abscess, etc., as well as all other actions associated with ED Physician Support. . Each physician will be spending 100% of time performing patient care services. Under the VA quality assurance surveillance plan, random review of 10 charts per provider a month will be done to ascertain that an adequate history and physical have to been done to determine diagnosis as well as that a medication reconciliation note has been done for each encounter. The expected performance bench mark is 90% or greater compliance. Physician productivity will also be monitored monthly or quarterly to determine that providers are seeing an average of at least two patients an hour when the demand is there. Physicians are expected to participate in ongoing CME in order to maintain their license and board certification, as well as complete all VAMC mandatory training in a timely manner. Averages of 16,000 to 18,000 patients were expected to be seen on a yearly basis. Provide Physicians who are proficient in, and are able to apply for, current privileges in all of the following: Wound closure and suturing, suture removal, Incision and drainage of abscess, Aspiration of cavities – thoracentesis, paracentesis, Insertion of peripheral and central venous lines, Insertion of chest tubes, Cooke catheters, Nasogastric tubes, nasogastric feeding tubes, Foley, Draw venous blood and perform arterial punctures for blood gas, Change gastrostomy tube and suprapubic tubes, Familiar with the working of vascular access apparatus such as those used for chemotherapy and be able to flush them when clogged. Emergency wound care, e.g. unna boot placement, wound vac. care, wound assessment. Emergency airway management including cricothyroidotomy, endotracheal intubation (including at least two live demonstrated endotracheal intubations during the past 12 months). Management of epistaxis with anterior and posterior nasal packing. Hemodynamic stabilization of patients prior to transfer or to admission. Joint needle aspirations. Handling of pediatric emergencies when necessary for visitors. Shall be willing, if necessary, to medically respond to cardiopulmonary arrests, and provide Endotracheal Intubation. Ordering of pertinent laboratory and radiology tests, initial diagnosis, computerized data entry of chart documentation and appropriate disposition of patients, with necessary consultative services within VA guidelines. Prescribe medications for patients according to appropriate clinical protocols and the established drug formulary within Hampton VA Medical Center. Provide professional evaluation and timely disposition of patients to include : Appropriate discharge of patients if acute hospitalization is not required, Admission to VAMC Hampton if medically indicatedTransfer patients to another facility/accept transfer patients from other facilities in accordance with the established policies and procedures of the VA facility mission to VAMC Hampton if appropriate. Provide routine Employee Health coverage for evening, weekend and holiday .Visits when Employee Health office is closed. Provide assistance with other emergencies outside of the ED to save life or limb when unavoidable.Manage an estimated yearly workload between 14,000 and 18,000 patients and have the ability to manage workload fluctuations on a daily basis. Work Schedule: Normal Work Hours: Normal Work hours: 8:00am to 4:30pm. Emergency Room Hours: Two (2) twelve hour shifts - 7:00 AM to 7:00 PM and 7:00 PM to 7:00 AM including all Federal Holidays and all Weekends. Qualifications: ED Physician support provided by Physicians who are board certified in emergency medicine (BC EM), have a minimum of 5 years experience in emergency medicine, preferably have 5 years of experience after American Board Emergency Medicine certification, are ACLS and BLS certified, and posses at least one current license in the United States.
