About the Position: Under general direction of the Family Medicine Medical Associate and the Triage clinical Manager the individual will be responsible for providing nursing services, medical triage of patients who call in or drop in for urgent care services in Family Medicine, Women’s Services, and Pediatrics. Individual will also provide direct patient care as needed.
Duties and Responsibllities: (Note the following are duties performed by employees in this classification. However, not all duties listed are necessarily performed by each individual in the classification. Employees may perform other related duties at an equivalent level.)
• Provides for routine and preventive primary, prenatal, chronic illness, urgent care, drop in/telephone triage and family planning care for patients, through regular clinica schedules, ordering diagnostic tests and preparing a plan for treatment necessary.
• Provides health education to patients regarding medical issues (i.e., asthma, diabetes, etc.,) and updates health education materials available in clinic as needed.
• Refer patients to outside specialty providers as needed, and follow up to ensure that patients’ medical and psycho-social needs are being met.
• Provides for nursing/clinical care of clients needing triage on call-in or drop-in basis which may include scheduling patients and performs nursing procedures (i.e., IV’s NST’s, etc.)
• Make suggestions for improvements in clinic flow, patient scheduling and service delivery.
• Participates in quality assurance chart reviews and audits.
• Collaborates professionally with interdisciplinary team members, including providers, health educators, medical assistants, registered nurses and case managers. Attends meetings as necessary.
• Consults with physicians on the management, assessment and treatment of specific cases as needed, and per protocol.
• Will be in both triage night call rotation and Saturday triage clinic.
• Other duties as assigned by supervisor.
