Description BACKGROUND: Lutheran Family Health Centers serves the ethnically diverse, medically underserved neighborhoods of southwest and central Brooklyn. Over 40 years, this Federally Qualified Health Center (FQHC) has evolved into one of the largest and most comprehensive community health center networks in the country and is uniquely positioned in the national healthcare reform movement. As a Level 3 Patient-Centered Medical Home as designated by the National Committee for Quality Assurance, LFHC includes the nation’s largest residency program with 90 dental centers in 17 states and Puerto Rico, as well as New York’s largest behavioral health program with HIV primary care services. In 2010, over 92,000 active patients made 550,000 visits to 9 full-time primary care sites. LFHC also operates 28 school-based health centers spanning four NYC boroughs. Complementing this system is an array of ancillary and diagnostic services, rehabilitation and early intervention programs, WIC nutrition programs, social work services, and across-the-board health promotion/disease prevention activities. Most recently, LFHC added a significant program serving 8,100 homeless individuals, generating an additional 44,000 visits per year. LFHC is governed by the Sunset Park Health Council, a consumer-majority board whose members are not only patients but also reflect LFHC’s service population with respect to racial/ethnic composition and socioeconomic status. The LFHC network serves as the ambulatory care resource for Lutheran Healthcare, a fully-integrated healthcare system comprising a teaching medical center, the health center network, home care, long-term care , managed Medicare and Medicaid programs, subsidized senior housing, and community development. PRIMARY FUNCTION: This new position was created to advance LFHC’s continuous care management, and engage in primary care medical practice transformation focused on an innovative model of patient care. The SVP will oversee all clinical and quality measurement and improvement and build a forward-thinking, innovative, comprehensive program to ensure continuous care management and measure and deliver high quality of care outcomes. This individual will focus specifically on the development of a quality outpatient infrastructure that is aligned with the internal and external environment around provisions of care. The SVP reports to the Executive Director of LFHC and will work with clinical and administrative leadership to develop patient-specific outcomes for care that will serve to focus on improving the health of the communities-at-large.
