The team includes 5 Primary Care/Geriatrics physicians, 10 Physician Assistants and 2 Nurse Practitioners Description of Practice: Office, Homebound, Long Term Care & Hospital work Practice Mix: 1/3 - Hospital, 1/3 Nursing Homes, 1/3 Office & Homebound/Hospices 1:5 Night Phone Call (shared equally among all physicians), PAs take nursing home call There are some Hospital rounds on weekends.
