PACE provides all needed preventive & primary care as well as coordinating acute and long-term care services so that its participants can continue living in the community. It is a non-profit, community based managed care program sponsored by Loretto in cooperation with St. Joseph’s Hospital Health Center. PACE CNY has been providing services since 1997. It is one of 66 PACE providers in the nation.
