Sutter Health-affiliated Gould Medical Group is seeking a BE/BC fellowship trained Family Medicine Physician or Internist with geriatric experience. The ideal candidate is a strongly committed, high quality physician who will provide and manage medical care for both short and long term medical group patients in local skilled nursing facilities.

Duties include:
Work closely with case managers to facilitate timely discharges and appropriate utilization
Evaluation upon admission
Monitor lengths of stay and implement treatment plans using best practice guidelines
Supervision of a dedicated nurse practitioner
Discharge planning and timely communication with referring physicians
Night or weekend call (telephone only) will be minimal and is shared with the Internal Medicine department
