Hospitalist will join an innovative group of physicians. As an integrated member of the care delivery team,
provide optimal medical care to hospitalized patients with commitment to quality and service in cost effective
fashion.
Responsibilities:
1. Responsible for meeting all of the MCS Job Standards described below.
2. Has ability to demonstrate knowledge and skills necessary to provide care appropriate to the age of the
patients served. The individual must demonstrate knowledge of the principles of growth and
development over the life span and possess the ability to assess data reflective of the patient’s status.
They interpret the appropriate information needed to identify each patient’s requirements relative to
his/her age-specific needs, and to provide the care needed as described in the department’s policies and
procedures. Age specific information is developed further in the departmental job standards
appropriate to the individual hospitalist.
3. Understands program vision and how their responsibilities are integral to the GEMCare Quality Care
Model so that comprehensive patient care may be achieved.
4. Committed to care of hospitalist program patients for entire work day so that provider is always
accessible to GEMCare patient care.
5. Accessible to program leader and management for entire work day so that communication is facilitated.
6. Accepts and adheres to outlined call responsibilities so that patient care is timely and efficient.
7. Participates in efficient verbal or written sign-outs so that quality of care during transition time periods
is ensured.
8. Sees patients in timely fashion in acute and skilled nursing setting so that there are no delays in patient
care.
9. Maintains effective communication with primary care physicians upon admission and discharge and as
appropriate, so that the transition to and from the outpatient setting is smooth and efficient, without
compromise of care.
10. Maintains effective and routine communication with inpatient case managers on a daily basis and as
appropriate, with appropriate utilization of their knowledge regarding legal and regulatory requirements
so that team approach is fostered.
11. Maintains effective and personal communication with specialists so that timely consultation is ensured,
involving management as appropriate
12. Maintains effective communication with patient and family members on routine and daily basis and as
appropriate, so that care plan is delineated and understood.
13. Ensure end of life issues are appropriately discussed with patient and family, and documented, so that
patient care is consistent with these wishes.
14. Documents advance directives in all patient charts, so that the patients’ wishes are clear.
15. Utilizes the Patient Care Module so that necessary patient information is easily accessible
16. Visits and notates on all patients on the date of discharge so that discharge is appropriate and
comprehensive.
17. Attempts to complete daily rounds on each patient by 1:00pm, so that there is time for problem solving
and return visits for the more acutely ill patient.
18. Completes comprehensive notes on a daily basis and discharge summaries in a timely fashion so that
hospital by-laws are upheld. Refer to Medical Record Protocol.
19. Ensures proper education is provided to the patient and/or patient family so that compliance with
impending discharge orders is ensured.
20. Completes comprehensive discharge orders, communicating needs with both patient and inpatient case
manager so that the transition to outpatient care is streamlined.
21. Proactively identifies patients at risk for re-admission, candidates for chronic disease management, and
creatively use resources so that the program may achieve optimal long term outcomes for each patient.
JOB STANDARDS
1. Support the mission, vision and strategic goals of Managed Care Systems, LP and the department.
2. Attend and participate in required meetings and developmental (training) activities. Maintains all
certifications or licensing required by the job.
3. Adhere to all MCS policies and procedures as well as departmental policies and procedures.
4. Maintain strict confidentiality at all times. Perform duties in a manner that ensures compliance with all
current laws and regulations (HIPAA, etc.).
5. Able to adapt to new concepts, ideas and changes in work processes and the work environment.
6. Perform all work in a safe manner and within established safety guidelines.
7. Must be able to concentrate and focus in a moderately noisy work environment.
8. Sign all medical records in timeline consistent with hospital requirements
