Admission and associated documentation in a timely manner,Conduct daily discharge planning conference, Daily rounds and electronic note on all patients, Facilitate transfers of critical patients to tertiary facilities, Collaborate with the midlevel provider when indicated, Complete discharge summaries and related documentation in a timely manner, The contract provider documents all patient encounters in a timely manner utilizing the computerized patient record system (CPRS), The contract provider is expected to have knowledge of medical diagnoses, treatment/intervention, medication effects/side effects and mastery of the theories, principles, concepts and methodologies acquired through medical practice,The contract provider is required to use sound judgment in the selection of treatment interventions, The contract provider is expected to possess knowledge of the special needs of age-specific patients, both male and female that range in age from 18 to 85 (and older), The contract provider will provide patient education as clinically needed, The contract provider will complete all “Clinical Reminders” in the computerized patient record system (CPRS) for each patient (with assistance of support staff), Work with the help of support staff to reasonably meet the needs of our co-managed care population.
