PRIMARY FUNCTION: A newly-created physician executive position, the Associate Director, Division of General Medicine for Ambulatory Services and Care Management will have the opportunity to build and lead an ambitious new program to ensure continuous care management and high quality of care outcomes and maximized institutional resources. The Associate Director and colleagues will establish and lead the institution’s Patient-Centered Medical Home (PCMH) inititative. The Medical Center’s goal is to achieve Level III Medical Home certification in 2010. The Associate Director will be an active clinician who will also provide administrative and academic leadership to the department, and oversee the activities of clinicians and staff in the Medical Center’s primary care outpatient offices. The Associate Director reports to the Vice Chair, Medicine/Senior Vice President, Clinical Integration & Affiliations, and the Vice Chair for Education/Program Director, Department of Medicine.
